Name and Date of Birth changes Important Information for Patients
If you need to tell us your name has changed or your date of birth detail is incorrect please bring the relevant documentation to support the change to the reception team. The staff will photocopy the documents while you wait and return them to you. We would ask you at this time to complete a short surgery form verifying both your old and new details to accompany the photocopy. The change can then be easily processed by our admin team.
Please note that we are unable to process a name or date of birth without sight of the relevant paperwork first.
