New patients from whatever background young or more senior are most welcome to register with our practice. Proof of entitlement to NHS care is however required & the completion of basic paperwork. New patients on regular medication are recommended to see either one of the doctors or nurse Kim prior to needing a repeat supply of meds.
Under the new GP contract all patients are registered with the practice and not assigned to an individual doctor’s list. Our patients remain free to see whichever doctor they prefer.
For any episode of illness however we would ask you to always try to see the same clinician, if possible. Our clinical team work closely together & difficult cases are regularly discussed
Medical treatment is available from the date of registration. Please contact reception for further information.
New patients living within our inner boundary are welcome to register. This process can even be started on line now and completed when you first visit us.
To give existing patients greater choice and recognising some may work close to the surgery, we can now keep you on our list when you move within our outer boundary so long as we feel this is appropriate bearing in mind your clinical history and likely ongoing medical/nursing needs. Do keep in mind those likely to need frequent home visits would usually be better served by a practice within a reasonable distance of the home address.
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct and to complete the pre-registration form.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is a link to this form at the bottom of the page which you can print out and complete. Please bring this completed medical questionnaire with you when you attend the surgery to sign your pre-registration form.
Please note we can not add you to the surgery's computer system with out a signed pre-registration form and a completed medical questionnaire. Therefore we request that you attend as promptly as possible to finalise your registration with the practice, which will allow us to access your previous medical records as quickly as possible.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off all the registration forms, fill them out and bring them in with you on your first visit to the practice.

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups. Open the leaflets in one of the following languages.
THE NHS DIRECT SERVICE referred to in the attached leaflets no longer exists, this has been replaced by the NHS 111 service. Just telephone 111 for all medical information.
If you need to tell us your name has changed or your date of birth detail is incorrect please bring the relevant documentation to support the change to the reception team. The staff will photocopy the documents while you wait and return them to you. We would ask you at this time to complete a short surgery form verifying both your old and new details to accompany the photocopy. The change can then be easily processed by our admin team.
Please note that we are unable to process a name or date of birth without sight of the relevant paperwork first.
We are trying to identify & support as many carers as we can.
Particularly those people who may be looking after a member of their family or helping a friend or neighbour with day to day tasks, don't really regard themselves as a carer and are undertaking this vital activity without help or support.
If you are caring for someone, we would really like you to let us know, so that we can ensure you receive all the support and information we can give, on topics such as benefit entitlement, access to respite care or maybe simply being there to provide a kindly ear when things get too much.
If you are a carer please complete the form in the carers Direct section on the home page of this website.
