If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness To Work’ Certificates.

If you have not been seen by a clinician for this illness you will need to arrange to be seen in surgery.

If, however, you have already been seen by a clinician about this illness, you may request a certificate. Please contact the surgery using this form.
