Healthcare Assistants

Health Care Assistants (HCAs) and part time Phlebotomists help us with health promotion activities and in monitoring those with long term conditions such as high blood pressure, asthma, diabetes and heart disease.

Care Coordinator

Mr Lincoln Abraham
We’re pleased to introduce Mr. Lincoln Abraham, our Care Coordinator and member of our Senior Management Team, who is a key member of our team. Care Coordinators work closely with the clinical team to support patients in managing long-term conditions and coordinating care across services, ensuring continuity and a personalized approach to health management.

Their role involves assisting with care planning, monitoring, and helping patients navigate healthcare services to improve outcomes and reduce hospital admissions.

Nursing Associate

Tracy Bayliss
Nursing Associate Tracy Bayliss is an integral part of our team, working closely with our nurses to provide a range of essential healthcare services. Tracy plays a key role in delivering nursing duties, including ECG monitoring and phlebotomy, ensuring high-quality care for our patients. Tracy supports our nurses in patient assessments, health promotion, wound care, and vital sign monitoring, bridging the gap between healthcare assistants and registered nurses. This valuable position helps enhance patient care by allowing greater flexibility within our clinical team.
